Complaints and Grievances

If you have a problem or complaint, please address your concern to one of our staff before you leave.  If you would like to contact us after your visit, please call or email of the following staff members, depending on the nature of your concern. If necessary, your problem will be advanced to center management for resolution.

If your concern pertains to confidential health information please call or send a message via our secure email portal. 

You have the right to have your verbal or written grievances investigated and to receive written notification of actions taken. If you do not agree with outcome or actions taken by our facility you can contact the Office of Healthcare Quality with a grievance in writing to Office of Healthcare Quality; Maryland Department of Health and Mental Hygiene; 7120 Samuel Morse Drive, Second Floor, Columbia MD 21046-8231 or by fax 410-402-8213.  If you have any questions you may reach them by phone at 410-402-8213.

If you have a concern with safety at our center and you do not agree with the outcome or action taken by the facility you may reach out to The Joint Commission.  You can file a complaint online or by mail at the address listed on their website. 

Medicare beneficiaries may also file a complaint with the Medicare Beneficiary Ombudsman, contact Medicare online or or call 1-800-MEDICARE (1-800-633-4227).
